The Five Wishes, advance directive addresses personal, emotional, and spiritual issues in addition to meeting medical and legal criteria for people who have had a medical crisis and cannot speak for themselves.
The document has been widely acknowledged and embraced by families,community groups, faith communities and medical and legal providers who want to plan for care in advance of a health crisis.
The Five Wishes advance directive created by the non-profit organization Aging with Dignity has been described as the "living will with a heart and soul".
